Each daycare business has its own individual policies and procedures. However, all daycare providers must issue standard childcare authorization forms in order to protect the children in their care as well as their daycare employees. The field trip or activity authorization form, gives the children in your care permission to leave the facility or participate in a special activity that is outside of the normal daily routine.
How to Create a Field Trip Permission Slip
There are some very basic elements that must be included in the form. These elements provide clear communication between the parents and yourself and, once signed and returned, helps limit your liability. Here are some things that must be included in the field trip permission slip:
Location:
The field trip consent form should contain the location of the activity. Provide details that contain the complete address and phone number of the location.
Date and Time
Include the date of the field trip and when the field trip begins and ends.
Transportation:
How will you get to and from the field trip? Include the mode of transportation involved (walking, car, bus etc.).
Cost
Be explicit about how much the field trip will cost the child. For example, if there is a charge for transportation or for the activity, provide this information. If there is a charge for transportation and a charge for the activity list them separately. If there is no charge at all, makes sure you explain that there is no charge.
Meals
On this section of the form you will provide information about whether or not lunch will be provided, if children need to bring their own lunch, or if there is a charge for lunch.
A Statement about Supervision:
Include a statement that discusses how you will supervise the children, such as “I understand that the center will take every precaution and care to insure my child’s safety. Adults who will provide proper supervision and will exercise every precaution to avoid accidents, in accordance with the state licensing regulations will accompany visiting groups”.
Statement about Field Trip Authorization Forms
It is important to let families know that they MUST sign and return the form in order to participate. You may use the following wording: “No child will be permitted to attend any field trip unless this form has been filled out in advance by the parent or Guardian and turned into the director.”
